In the 3 dimensional equation (the huge ugly one), you forgot to put the "+" sign between the partial derivative of the wavefunction with respect to r, and the fraction which includes sines and partial derivatives with respect to theta..!! Hope this makes sense! I couldn't describe it more acurately..
